In Part 12 we ended up showing this picture and asking you if you could guess what it was. Here is a full picture. Yes, it is a manufacturing defect which, to use Dave's terms, is a 'fibre tear'. The glass insert includes vertical lines which are made to look like metal but seem to me to really be a type of plastic.
